On Wednesday, April 28, 2021, the Nevada Supreme Court issued an order in ADKT 0571 adopting standardized forms related to the protection of children from abuse and neglect. The approved forms can be found in Exhibit A attached to the order.
Electronic templates of the forms are available to authorized users online; Requests to become an authorized user must be emailed to cip@nvcourts.nv.gov.
Use of the standardized forms becomes effective June 27, 2021, and use of the forms will become mandatory for all child abuse and neglect cases on January 1, 2022. With the issuance of this order, ADKT 0571 is now closed.
