Submit a Department of Motor Vehicles Report from every state in which licensed to drive during the past five (5) years. (SCR Addendum 1, Paragraph 11) The driving report must be validated by the DMV, or you must provide a confirmation number or transaction number to prove authenticity. All DMV reports combined must cover the span of at least the past 5 years.
