Computer Registration Form Submission

Applicants who have selected to take the exam by computer must submit one original copy of the Computer Registration Form. The Computer Registration Form must be signed in BLUE INK ONLY. Failure to comply with all the registration requirements will result in the applicant being required to handwrite the exam in the computer room.
